"Taking CAP State-Wide," (1995-1997) was very successful in meeting
its goal of making North Carolina's Community Alternatives Program
for Disabled Adults (CAP/DA) available across all 100 counties.
CAP/DA is North Carolina's Medicaid waiver program for home and
community based services that serve as an alternative for nursing
home placement for the low-income frail elderly and disabled adults.
Prior to the advent of Aging at Home II, 12 of the 100 counties
had not yet established a CAP/DA program. Thanks to a generous grant
from the Kate B. Reynolds Charitable Trust and the program management
and technical assistance provided by Duke LTC, Aging at Home II
was able to provide all 12 counties with the start-up funds, training,
and assistance required to serve the needs of the low-income frail
elderly and of disabled adults who qualified for nursing-home level
care, but wanted to age in place.
